| Introduction | Cytochrome P450 3A4 (CYP3A4) is an enzyme involved in the phase I metabolism of exogenous small molecules (xenobiotics). CYP3A4 is a ubiquitously-expressed monooxygenase that uses one molecule of dioxygen and a molecule of NAD(P)H to transfer a hydroxyl group onto a substrate and create a water molecule. CYP3A4 also has epoxygenase activity; it can create three-membered rings (where one atom is oxygen) on arachidonic acid, which leads to the proliferation of certain cancers. There are many known substrates for CYP3A4, including cyclosporin, codeine, simvastatin, erythromycin, and caffeine. Bergamottin is a well-known CYP3A4 inhibitor; this chemical is found in grapefruit juice and is one of the main reasons why the fruit has such a large number of contraindications. There are no diseases associated with CYP3A4 mutations, however the metabolism of various drugs can be greatly affected by different polymorphisms. |
